If you don't want to submit your information electronically, you can print a copy of the form here and fax or mail it to LHPAC. All information must be received by July 7, 2008. Changes only can be made until July 21, 2008.

Special Requirements

Do you need access to electricity for fountain pumps or for lighting that is integral to a piece (ie. neon signs)? Note: We do not allow general display lighting. Yes   No

Do you have large pieces that need to be displayed outdoors? Yes   No

Do you have any unusually heavy pieces (ie. more than 2000 lbs.)? Note: We have forklifts available to move heavy pieces, but we do not have cranes. Yes   No

Do you have any pieces that need to be mounted on a wall? If so, how many wall units will you need?  Please remember our wall units are 4'x 8' and they are placed along the tent perimeters. We do not construct special sized walls for your pieces.
