Case Management clients generally have multiple long term needs with case files established and monitored by staff due to the in-depth amount of work required to assist them. On-going support services to people with disabilities who have case files include:

These services help clients manage their lives and the services they need in order to live independently.

Case Management addresses multiple, complex needs specific to persons with disabilities. Case managers act as a bridge between clients and services, working directly with clients to identify needs, assist them in gaining access to appropriate services, and support them in developing the skills to live or to continue living independently. The level and type of assistance provided by case managers is different for each consumer. Many issues that are addressed and resolved involve basic needs such as locating affordable housing, obtaining food, and finding emergency financial assistance. The added challenge with disabilities may include basic needs for wheelchair accessible affordable housing, special dietary food, and financial assistance for medications, medical supplies, and essential equipment, the lack of which may place the person in a life-threatening situation. In addition to basic survival, there is a multitude of other needs and issues consumers have as part of living with disabilities. One may need a self-advocacy trainer, someone else a financial advisor, and another a community resource coordinator.
